What is the process to participate?
Using the contact form above let the project know how you would like to participate.
You will be contacted via email with project background, a legal release, and short questionnaire about your diagnosis and treatment.
We will schedule an hour interview in person or online.
We will reach out annually through the information gathering phase for 30 minute interviews.
Can I see a copy of the release and initial questionnaire?

How will my information be used?
Initially the information will be used to inform a monthly breast cancer survivorship support group to better serve survivors.
The information will also be used in a published book to support and guide survivors and their families.
If you chose to be a resource for another survivor your contact information can be shared with the survivor for ongoing support.
Will you be collecting Personal Heath Information (PHI)?
Participants will be asked and have the choice to provide PHI including diagnosis and treatment details. This information is important because treatment depends on diagnosis and can contribute to life after breast cancer.
Who are you?
I am a breast cancer survivor. I was diagnosed in February 2020 at the age of 45 with Stage 3b hormone positive, HER2 negative breast cancer. I went through treatment during COVID that included AC/T chemo, DMX, radiation, 2 reconstruction surgeries, a year of Tamoxifen and now Letrozole (AI).
I volunteer to facilitate a monthly Survivorship Support Group for Breast Friends Oregon, who were invaluable to me during my journey.
I am a civil servant, pet parent, artist, and continuous learner. I am single and have an amazing community of friends.
